This page lets you share important information about incidents or concerning behavior in the Florida Southern College community. When a report has been submitted, it will be reviewed in confidence by the appropriate college official.

Important Information About Anonymity
The College accepts anonymous reports. However, anonymous reports may limit our ability to investigate thoroughly or take appropriate action because investigators may be unable to verify information, obtain additional details, identify witnesses, or notify you of available resources or the report outcome. All reports are documented regardless.
Per Title IX rules, complaints of discrimination based on gender must be formally filed (not anonymous) to be investigated.

Use this form to report any possible violations of Florida Southern College's Sex Discrimination and Sexual Misconduct Policy, such as all forms of non-consensual sexual activity and unwelcome sexual conduct, including: sex discrimination, sexual violence (rape/sexual battery/sexual assault), sexual harassment, relationship violence (domestic violence and dating violence), stalking, harassment or violence based on sexual orientation, gender identity or gender expression, and other forms of sexually exploitative behavior.

Use this form to report any possible violations of Florida Southern College's Non-Discrimination and Harassment Policy, such as discrimination based on race, creed, color, national origin, gender, gender identity, sex, sexual orientation, marital status, pregnancy, religion, age, disability, political opinions or affiliations, genetic information, veteran or military status, and any other protected category under applicable local, state, or federal law.

Use this form to report any possible instances of hazing. It is a violation for students, faculty, staff, or alumni to engage in any activity that may be construed as hazing on or off campus. Hazing is any action taken or situation created, that produces mental, emotional, or physical discomfort, embarrassment, harassment, or ridicule. This applies to all student organizations, including athletics teams.

Timely reporting allows the College to respond quickly, assist those affected, assess potential threats to the campus community, and take appropriate action to prevent further incidents.
