Reporting Experiences

Florida Southern students are required to report all internships, professional shadowing experiences, research roles, and other career-related experiences to Career Services. This applies whether or not you're earning academic credit.

Reporting lets us support you during the experience, document your work for future employers and graduate schools, and track outcomes across the College.

If you're earning academic credit, all paperwork must be submitted and approved before the semester starts.

Report an Experience (Learning Contract and Liability Waiver)

Required for all Florida Southern students  completing an internship or career-related experience. Doctor of Physical Therapy students see below.

Forms and Waivers

Liability Waivers for DPT Students — required for Doctor of Physical Therapy students.

Employer Evaluation of Student — completed by your supervisor at the end of the experience.

Tracking Your Experiences for Graduate and Professional School

Report your experience to Career Services, then keep your own record of it. You're responsible for tracking your own hours, and you'll draw on these notes when you update your resume, write personal statements, and complete graduate and professional school applications. Two years later, you will not remember the details that make an application essay work.

Track as you go:

  • Start and end dates
  • Supervisor name and contact information
  • Average hours per week
  • Specific procedures and projects, using correct medical or scientific terminology
  • Your reflections: the moments that went well and the ones that didn't, what you learned about yourself, memorable or difficult patients and clients, new technologies or methods you encountered, and what you noticed about the field
